All campers are required to fill out the Health, Info, and Permission forms below. Please note that some camps require the other two forms. These forms were due to the Leadership and Learning Center by June 1, 2013.
If you have not returned your forms yet, please bring them to camp on opening day.
If your daughter has any special medical needs such as allergies/food restrictions or other needs, it is very important for you to:
- For Council Camps – Call the camp nurse “Red” at Camp McMillen in June at 1-877-637-6279 or at Camp Logan in July at 1-888-985-6426 the week prior to her camp session at the latest, preferably 2 weeks ahead.
- For volunteer led community camps please contact the director for your camp for more information.

This form provides the information we need to give your camper the best experience possible. No campers will be admitted without this form. - Challenge/Adventure Form
Suggested for all campers
Campers who would like the opportunity to participate in the challenge course or climb the rock wall must complete the Challenge Adventure Program Participation Agreement. The rock wall is only available to Girl Scout Juniors and above. - Climbing Wall Release Form Campers who would like the opportunity to participate in the challenge course or climb the rock wall must complete the Challenge Adventure Program Participation Agreement. - Horseback Riding Permission Form
Required for Horseback Riding programs suggested for all Camp Logan campers
Campers who sign-up for a Horse Camp session will need to have this form on file. Girls, attending Camp Logan, not signed up for Horse Camp may have the opportunity to choose a pony ride as an activity. No girls will be permitted to ride without this form.
